Stop Work Authority
To begin with, each and every employee from the owner of the company to an employee starting his first day on the job has the authority and responsibility to stop unsafe work. Stop Work Authority is discussed every day from the beginning of the Job Safety Analysis, signing in visitors to location, to each task we perform at the job site.
Job Safety Analysis
The Job Safety Analysis (JSA) is an important tool utilized by our rig crews and supervisors. They begin each working day by conducting a JSA, reviewing the JSA during visitor orientation to location, and updating the JSA when there is a change in activity. 4J Well Service requires that all employees and visitors must first review our current JSA, be orientated to the hazards associated with the tasks being performed, and be aware of any emergency protocols such as fire extinguisher placement, SCBA placement and Muster Areas.
Behavior Based Observation and Inspection
4J Well Service has recently adopted a behavior based observation and inspection program. This program consists of a checklist of items that are evaluated by either a supervisor, manager, or fellow employee. The key component of this program is discussing the findings with the crew involved and also reviewing any trends that are seen throughout the audits as seen below.
Near Miss Reporting / Incident Reporting
4J Well Service stresses to its crews the importance of Near Miss Reporting as well as incident reporting. Near misses are not looked at as “getting written up” but as an important tool to educate ourselves about what can lead to an accident and learn how to prevent the near miss or incident from occurring again. Near misses are looked at as leading indicators as to how well a crew understands the hazards and actions that may have occurred which could have led to even a minor incident. All near misses and incidents are reviewed and discussed during our Company Wide Safety Meetings.
Weekly Rig Inspection
Each Rig crew with their supervisor is required to perform a weekly rig inspection. These inspections include the categories of Rig and Wellhead Fires, Derrick Fall Hazards, Rig Turnover or Collapse, Power Tong Hazards, Employee Fall Hazards, Pump-Jack Hazards, Struck-By Hazards, PPE, First Aid, and Falling Tubing or Rods. These inspections are turned into company management on a weekly basis to be reviewed. Items noted are discussed and addressed either during the inspection, or for larger items they are discussed during the next Monday, Wednesday, Friday supervisors meetings.
Safety Meetings/Training
4J Well Service holds a variety of safety meetings from the daily JSA/toolbox meeting, the monthly Companywide Safety Meeting, to Supervisor/Management semi-daily morning meeting. All employees are also trained in the PEC SafelandUSA orientation that is designed specifically for the US Onshore E&P industry. The training covers all the aspects of working in the industry from the basics of back safety and defensive driving to more hazard specific topics such as Lock Out/Tag Out, Respiratory Protection, and H2S Safety.
